Usually a vet will not do surgery if they feel they do not have the surgical skill or equipment needed for a good outcome (I have been lucky enough to meet vets like that, and unlucky enough to meet vets that didn't). Usually in that case, the vet will refer you to a surgical specialist or other vet.

The placement of the stones can also determine whether surgery is feasible: stones in the ureters (tubes leading from kidneys to bladder) are a very poor surgical risk, as are stones in the kidneys. Stones in the urethra (tube leading from bladder to "outside" can sometimes be flushed or gently removed under local anesthesia.

How old is Herbie? She has an advantage: being female, there is a great deal less "plumbing", and sometimes it's possible to pass the stones.

Diet modification and increased water intake will be needed to help manage the condition whether she has surgery or you try one of the non-conventional therapies (herbs, etc.)

Antibiotics and metacam is the standard management for newly-diagnosed stone patients. Is Herbie passing any blood?
